This puzzle requires you to count all possible openings, not just the visible cuts!
The holes needed to wear the shirt are: 1. Neck opening, 2. Bottom opening, 3. Two armholes (sleeves). That's 4 holes.
Then, there are two visible holes cut into the fabric. Since these holes pass all the way through the shirt (front and back), each visible hole counts as two (entrance and exit). 2 visible holes * 2 sides = 4 functional holes.
